1. This Notice Describes How Medical Information May Be Used and Disclosed
This Notice of Privacy Practices (“Notice”) describes how protected health information (“PHI”) may be used and disclosed by healthcare providers who deliver services through Revano Life, LLC (“Revano”) and how you can access this information.
This Notice applies to PHI that is received, created, or maintained by independent licensed healthcare providers and, in limited circumstances, by Revano acting as a business associate.
2. Your Rights Regarding Your Health Information
You have the right to:
A. Access Your Records
You may request access to your medical records maintained by your healthcare provider, subject to applicable law.
B. Request Corrections
You may request correction of your medical information if you believe it is incorrect or incomplete.
C. Request Confidential Communications
You may request that communications be delivered in a specific way (e.g., email, phone, mail).
D. Request Restrictions
You may request restrictions on how your information is used or disclosed. Providers are not required to agree to all restrictions.
E. Receive an Accounting of Disclosures
You may request a list of certain disclosures of your PHI made by your provider, as required by law.
F. Obtain a Copy of This Notice
You may request a paper copy of this Notice at any time.
3. How Your Health Information May Be Used and Disclosed
Your PHI may be used and disclosed by healthcare providers for the following purposes:
A. Treatment
To provide, coordinate, or manage your healthcare, including diagnosis, treatment, and prescriptions.
B. Payment
To obtain payment for healthcare services provided, including insurance claims processing, billing, and collections.
C. Healthcare Operations
For administrative, quality improvement, compliance, and operational purposes.
4. Uses and Disclosures That May Occur Without Your Authorization
Your PHI may be used or disclosed without your authorization in certain situations, including:
- Public health reporting
- Reporting abuse or neglect
- Health oversight activities
- Legal proceedings or court orders
- Law enforcement requests
- Serious threats to health or safety
- As otherwise required by law
5. Revano’s Role
Revano may, in limited contexts, support administrative and technical functions on behalf of healthcare providers and operates under Business Associate Agreements where required.
In such cases, Revano is required to safeguard PHI in accordance with HIPAA and applicable Business Associate Agreements (“BAAs”).
Revano does not independently make clinical decisions or determine treatment.
6. How We Protect Your Information
We implement reasonable and appropriate administrative, technical, and physical safeguards to protect PHI, including:
- Access controls and authentication systems
- Encryption of sensitive data where appropriate
- Secure hosting and data storage systems
- Employee access restrictions and training
However, no system can guarantee absolute security.
7. Uses Requiring Your Authorization
Certain uses and disclosures of your PHI require your written authorization, including:
- Marketing purposes in some circumstances
- Sale of health information
- Any use not otherwise permitted by law
You may revoke your authorization at any time in writing, subject to legal exceptions.
8. Breach Notification
If a breach of unsecured PHI occurs, affected individuals will be notified in accordance with federal and state law.
9. Changes to This Notice
We reserve the right to change this Notice at any time. Updated versions will be posted on the Revano platform with a revised effective date.
10. Complaints
If you believe your privacy rights have been violated, you may file a complaint with:
- The healthcare provider involved, or
- The U.S. Department of Health and Human Services (HHS)
You will not be retaliated against for filing a complaint.
